Call for Nominations for Annual Awards

Each year, at the annual meeting, the North Dakota Family Based Services Association is pleased to recognize individuals and programs that have made a difference in the lives of North Dakota families. The awards are given based on supporting family based programs in their field of practice, enhancing the visibility of family based services in North Dakota and promoting the mission of the NDFBSA. Professionals and families in North Dakota can nominate deserving individuals and programs for consideration. Please take a moment to think about whom you could recognize through these awards:
  • Program Award – for outstanding or innovative family services or for a program that fills a gap in the existing services.
  • Provider Award – for an individual who has exemplified the Family Based Services philosophy in their professional practice.
  • Lifetime Membership Award – for an individual who has provided a minimum of 7 years of meritorious service and/or employment in support of Family Based Services philosophy.
  • Special Recognition Award – Person or program that is innovative and responsive to immediate needs or current issues in keeping with a Family Based Services philosophy.
  • Presidents Award – a discretionary award, used by the NDFBSA Board to identify individuals or programs that excelled in providing family based services over time.

The criteria for nomination of an individual or program include:

  1. Outstanding and significant support of Family Based Services philosophy and mission.
  2. Exceptional leadership
  3. Significant contribution to community
  4. Demonstration of creativity in pursuit of Family Based Services
  5. Innovative use of resources, personnel and/or collaboration

If you would like to nominate someone or a program, please write a letter of nomination that addresses how the individual or program exemplifies the criteria above and submit with three additional letters in support of the nomination that also address the criteria.

Completed nominations will be accepted until February 1, 2008.

Nominations will be reviewed by the NDFBSA Board at the February Board meeting. Individuals or programs may be recognized more than once for exceptional distinction in their field or service in promoting Family Based Services.
